This RMA form is for
Warranty Replacement of defective or damaged insoles
only. For refund requests

To be eligible for a
Warranty Replacement you must fill out the RMA form on this page
and agree to the Warranty Replacement Terms of
Service. Once we receive by postal mail
delivery, your insoles,
warranty card or dated Proof of Purchase document,
applicable payment, and a copy of the RMA form results
(found in your email once you hit the submit
button), you will receive a
replacement pair of insoles within 2-4 weeks.
You must
postmark your insoles on or before 365 days (One
Year) from the
date of purchase for insoles covered under the One
Year Warranty, 730 days (Two Years) from the date of purchase
for insoles covered under the Extended Two Year
Warranty, or 1095 days (Three Years) from the date of purchase for
insoles covered under the Extended Three Year
Warranty. The date of purchase is hand written on
the warranty card (Inside the Original Insole Package). Customers
who purchased the insoles at a retail outlet who
do not have a hand written date on the warranty
card must include a copy of a sales receipt or
a copy of the credit card statement showing FOOTRELIEFINSOLES
and a date
in the transaction description.

copy of the RMA form results by email. You
must print the RMA form results from your email
and send a copy with your
warranty replacement request.
Please check your
spam and junk folders before contacting us if you do not receive
a copy of the completed RMA form results.
